The PACT Project
The PACT Community Health Worker Program strive to improve clinical outcomes and quality of life for HIV-positive patients who are struggling with their health because of any number of individual and structural barriers including poverty, mental illness, social isolation, or substance abuse. The PACT project trains and employs community members to check in on their HIV patients daily/weekly, making sure they attend medical appointments, take their medications, and have access to other essential needs and social services. PACT's community-based approach has achieved measurable success both in improving health status and in reducing costs. PACT's clients see a huge improvement in their CD4 counts after spending time in our intentions.
